Applicants and candidates who require special assistance or need to respond to specific requests from The Strategy Institute may find a set of important forms useful for clarifying or providing additional information about their application or candidacy.

• If you have received a notification that your application has been chosen for a random candidacy audit, you must complete the Candidacy Audit Response Form provide any additional information requested in the candidacy audit notification, and submit it here.

• If you don’t meet the eligibility criteria for the TSI certification program but still want to be considered, you can fill out the Candidacy Consideration Request Form. This form is also for applicants who were not accepted and want their application to be reconsidered. You have five business days from the communication received from TSI to submit your request. To complete the form, you'll need to provide additional information requested in the candidature disapproval notification and submit it here.

To file a complaint, kindly complete the Complaint Form, and submit it here. We kindly request that you provide a comprehensive summary of the allegations alongside any supporting documentation and information.
